Enter a New Patient using the Patient Registry Form
In order to create a new patient
using the Patient Registry Form, you must bring the Patient
Registry Form up on the screen. To do this, click on the Form button
to view the Form Button Menu. On the Form Button Menu, you
will see among other options, "Patient Registry".
Click on Patient Registry.

To create a new patient record,
press the New button
on the toolbar:
When the New button is clicked, you will notice the data fields on the form change to yellow and white. Non-gray fields indicate that the form is ready for you to input your patient information. When a field is required in order to successfully reconcile an OHIP bill with the Ministry of Health, it is colored yellow. While you may skip yellow fields if you don't have complete information, bills for patients with skipped yellow fields will not be submitted to OHIP until all the required information is entered into the computer.

If you have a photograph of the patient you wish to attach to the patient file, there are two ways to incorporate it.
If you have a compatible camera device directly attached to the computer, the Photograph Now button should be available. When you press the Photograph Now button the camera device will take a still picture and paste it into the Patient Registry photo frame.
If you do not have a direct attached camera, but have a photograph that has been scanned into the computer, open the file in any photo editing software, and copy the photo to the clipboard using the Copy option which is usually found on the Edit menu, or sometimes on the right-mouse button menu. Then go back to Visual Practice and press the Paste Picture button, and it will paste it into the Patient Registry photo frame.
When you have completed entering
all of the patient data necessary, press the Save button
on the
toolbar.
